This application is for individuals who are interested in being accepted into our Family & Peer Workforce Development Program as a direct service worker and/or supervisor. We ask that all individuals participating in our program complete this application. Completion of this application will support our abilities to learn more about your background, experience, and qualifications required for training and specialty workforce development internship and apprenticeship opportunities.

This application should take approx. 5-10 minutes to complete. You will be asked to provide information including your contact information, race/ethnicity, education, citizenship, computer skills, lived experience as individual with opioid addiction, substance use and/or mental health, parental, family and child experience, previous training programs, transportation options, and any special accommodations you may need to participate. You will also be asked to write a short paragraph on what recovery means to you. After you complete the application, you will receive a confirmation that the application was received and will be contacted for a follow-up interview. The interview will determine selection for specific training, internship, and apprenticeship opportunities.
